ROUTINE HOME CARE
Care is provided in a home setting, this could be your own house or a facility. Our caring staff will visit you, and Life Springs will provide all medical equipment, supplies, and medications related to the primary diagnosis.

CONTINUOUS CARE
When a medical crisis develops which cannot be managed by the family, and symptoms cannot be controlled with scheduled intermittent visits, Life Springs will coordinate and manage continuous home care until patient’s condition stabilizes. Care is available for as much as 24 hours a day as necessary to maintain patient at home.

GENERAL INPATIENT CARE
When care is needed for the treatment of acute medical and/or psychosocial symptoms related to the terminal illness, inpatient care on a short-term basis is available at a contracted skilled nursing facility or an acute care hospital

RESPITE CARE
If needed, Life Springs can provide a brief inpatient stay to allow a caregiver or family member a “respite” or break.

HOSPICE PHYSICIAN
Works with patient’s primary physician and is available for pain and symptom management consultation.

SKILLED NURSE
Monitors the patient's physical health and communicates any changes or medication needs to your primary physician. Completes a physical assessment during each visit, and is expert in pain and symptom management, nutrition and wound care.

SOCIAL WORKER
Provides emotional support and helps secure needed assistance with community resources such as advanced directives and funeral arrangements to patients, families, caregivers and friends.

VOLUNTEER
Well-trained to listen and provide emotional support.

CHAPLAIN
Specially trained and dedicated to respecting patient’s varied spiritual beliefs. They are or your side to support your illness, grief and loss.

HOSPICE AIDE
Provides personal assistance to patients such as bathing, oral hygiene and other care needs.
